Jessica Nemeth, Hypnotherapist
The undersigned Client acknowledges that he or she has been informed of the following information:
Hypnotherapist agrees to provide professional services in accordance with acquired training and experience giving undivided attention during scheduled consultations to facilitate Client's benefits. Hypnotherapist's work is Client-centered. Services provided utilize induction of hypnosis, and methods and principles used to help clients discover their inner creative abilities to develop positive thinking and feeling and to transform undesirable habits and behavior patterns. Therapeutic goals are to achieve freedom from restrictive thought and belief systems, to assist in solving personal problems, developing motivation and achieving goals. Client may be taught the use of self-hypnotic techniques to assist in achieving goals and resolving issues that have been mutually agreed upon by Client and Hypnotherapist.
Hypnosis is not a state of sleep, but is a natural state of mind that can produce extraordinary levels of relaxation of mind, body and emotions. The principles and theories upon which hypnotherapy is based are accessing and utilizing the power of one's inner resources. Hypnosis can transcend the critical, analytical level of mind, and facilitate the acceptance of suggestions, directions and instructions desired by the Client. The therapeutic use of hypnosis can also elicit information and insights from the inner mind. The Hypnotherapist utilizes interviews, discussion and hypnotic methods dealing with underlying issues whenever appropriate, with the goal of achieving effective and lasting results.
Services to be provided do not include the practice of medicine, as Hypnotherapist is not a licensed physician. These services are non-diagnostic, and are complementary to the healing arts services that are licensed by the state. The California State Legislature has determined that state licensing may not be conferred upon an occupational group for purposes of status or prestige. The primary purpose of licensing laws for legally defined Healing Arts and Mental Health professionals is to protect public health and safety. Accordingly, Hypnotherapists are not issued licenses by any state governmental agency to engage in their professional services.
I, ____Jessica Nemeth_________________________, have acquired the following education, training, experience, and qualifications to perform the services

Along with your payment, you will be asked to give your contact details. This will then form an agreement between you and the Company for your therapy.
THERAPY
Successful sessions rely on your commitment and motivation. When you agree to partake in your therapy sessions, you, as the client are considered a “co-therapist”, which means you agree to be fully involved and committed to the process, which may include self-work if necessary. Ongoing sessions will always be discussed with you and you will have the right to express your preferences regarding your treatment plan and timeframe.
You are free to end or withdraw from therapy at any time. The cost of any sessions paid for in advance but not used will be refunded on request, subject to my cancellation policy. Refunds will reflect the actual price paid if special offers have been used.

REFUND POLICY
Hypnotherapy services are not eligible for refunds as the time has been allocated. However, we may offer a free session if there is an issue or concern with the session.
The hypnotherapist facilitates change by guiding the use of hypnotic suggestions. The hypnotherapist cannot control or force the client to change or do anything they do not want to do. At the end of each hypnotherapy session, the hypnotherapist will ask if the client is satisfied. If the client feels doubt or apprehension during or immediately following a hypnotherapy session, it is in the client’s best interest to communicate with the hypnotherapist as soon as possible.
If the hypnotherapist has any concerns throughout the client session, the concerns will be discussed with the client to determine the recommended course of treatment. The client has the option to take the recommendation or to not go ahead with future sessions. If the client has paid in advance for hypnotherapy sessions, the client has the option to not go ahead and the client will be refunded the amount that was paid in advance.
Although Jessica Nemeth Hypnotherapy has had great results, results cannot be guaranteed and may vary based on the mindset of the client, the desire to change, and the commitment to follow instructions during and after the session.
DISCLAIMER
We will always do our best to help you achieve a successful outcome to your therapy. However, you accept that an agreement to work on an issue does not mean that a particular outcome to therapy can be guaranteed. Fees are payable for your sessions and the time and expertise of the therapist, regardless of the outcome. Results may differ from person to person.
With pain management sessions, if you wish to reduce or stop any medication then medical support must be obtained. This appointment does not replace medical advice and assistance.
